HMOs are organized healthcare systems that are responsible for both the financing and the delivery of a broad range of comprehensive health services to an enrolled population. HMOs act both as insurer and provider of healthcare services. They charge employers a fixed premium for each subscriber. An independent practice association (IPA)-model HMO provides medical care to its subscribers through contracts it establishes with independent physicians. In a staff-model HMO, the physicians would normally be full-time employees of the HMO. Individuals who subscribe to an HMO are often limited to the panel of physicians who have contracted with the HMO to provide services to its subscribers.

Preferred Provider Organizations

Preferred provider organizations (PPOs) are entities through which employer health benefit plans and health insurance carriers contract to purchase healthcare services for covered beneficiaries from a selected group of participating providers. Most states have specific PPO laws that directly regulate such entities. Common characteristics of PPOs include:

•   Select provider panel

•   Negotiated payment rates

•   Rapid payment terms

•   Utilization management (programs to control the utilization and cost)

•   Consumer choice (allows covered beneficiaries to use non-PPO providers for an additional out-of-pocket charge [point-of-service option])

In PPOs, a payer, such as an insurance company, provides incentives to its enrollees to obtain medical care from a panel of providers with whom the payer has contracted a discounted rate.

Exclusive Provider Organizations

Exclusive provider organizations (EPOs) limit their beneficiaries to participating providers for any healthcare services. EPOs use a gatekeeper approach to authorize non–primary care services. The primary difference between an HMO and an EPO is that the former is regulated under HMO laws and regulations, whereas the latter is regulated under insurance laws and regulations. Characteristics of EPOs include the following:

•   Primary care physicians are reimbursed through capitation payments or other performance-based reimbursement methods.

•   Primary care physicians act as gatekeepers. (Pozgar 514)

Part II: Legal and Ethical Issues

Each state has statutes that govern the practice of medicine in that state. These are called medical practice acts and are meant to protect the health and safety of the public (Fremgen, 2012). Each state legislature establishes a state medical board that has the authority to control the licensing of physicians. While some slight differences exist from state to state, in general, these practice acts define who must be licensed to perform certain procedures (Fremgen, 2012). These acts also specify requirements for licensure; the duties of the licensed physician; grounds on which the license may be revoked or taken away; and reports that must be made to the government or other appropriate agencies (Fremgen 2012).

When healthcare workers hold patients’ lives in their hands, they must know the right thing to do when complications arise (Aiken, 2009). Most healthcare training focuses on what steps should be taken when medical emergencies or clinically related problems arise (e.g., apply pressure to stop bleeding).

In addition to clinical and medical complications, healthcare professionals must also deal with complicated ethical dilemmas (Aiken, 2009). For example, if a person who is bleeding is HIV positive, or if someone did not want heroic measures taken to save his or her life, should rescue breathing be started? These are ethical dilemmas, and the healthcare worker entrusted with the well-being of patients must know or be able to discern the right and the wrong thing to do. Ethics helps determine the right thing to do in the case of a moral dilemma (Aiken, 2009).

Week 5 – Sustainability (graded)

The discussion for this week is focused on sustainability. Will the change that you initiated for your DNP Project be sustained, modified, or discontinued? How is that being determined? What plans have you made for your exit? Will you continue work related to your practice problem? What are your next steps?

Week 6 – Portfolio (graded)

The discussion for this week is focused on the experience of developing a professional portfolio. How do you anticipate using your professional portfolio? As you reflected on your career while developing your professional portfolio, you likely reviewed your strengths and opportunities for growth. Please share anything you are comfortable with as a result of that self-evaluation. Also, share your long-term career goals and your plan for achieving those goals.

Week 7 – Transformation (Required, not graded)

The discussion for this week is focused on your transformation. As you complete your DNP Project final paper and presentation, and your portfolio, share with your instructor and your peers how you have been transformed through your entire DNP program experience.

Part I: Pamphlet

  1. Develop a pamphlet to inform parents and caregivers about environmental factors that can affect the health of infants.
  2. Use the “Pamphlet Template” document to help you create your pamphlet. Include the following:
  3. Select an environmental factor that poses a threat to the health or safety of infants.
  4. Explain how the environmental factor you selected can potentially affect the health or safety of infants.
  5. Offer recommendations on accident prevention and safety promotion as they relate to the selected environmental factor and the health or safety of infants.
  6. Offer examples, interventions, and suggestions from evidence-based research. A minimum of three scholarly resources are required.
  7. Provide readers with two community resources, a national resource, and a Web-based resource. Include a brief description and contact information for each resource.
  8. In developing your pamphlet, take into consideration the healthcare literacy level of your target audience.

Part II: Pamphlet Sharing Experience

  1. Share the pamphlet you have developed with a parent of an infant child. The parent may be a person from your neighborhood, a parent of an infant from a child-care center in your community, or a parent from another organization, such as a church group with which you have an affiliation.
