Identify and justify the top five objectives for nursing care

People aged 65 years and older constitute 50% of health service users. As people age, they experience changes to their physical and mental function, and can experience many social changes in terms of family and career. For those requiring health services, these changes are further complicated by illness (Moyle, et al., 2014).

The aim of this assignment is to apply the theoretical knowledge gained from modules 1 and 2 of this course to a case study that you have encountered in a Bachelor of Nursing clinical placement. Examples of case studies may include (but are not limited to) the range of prevalent conditions examined in module 1 and 2:

· Falls and fractures

· Incontinence

· Delirium

· Alzheimer’s Disease and other dementias.

TASK INSTRUCTIONS:

You will need to describe the issues in your case study, and then undertake a process of analysis in order to generate a comprehensive nursing plan of care. Practising this process of analysis will help you to feel more confident in providing nursing care to an older person in the clinical setting.

Your case study must:

1. Reflect upon the impact that various stereotypes of older people may have on the older person and their nursing care;

2. Recommend the nursing requirements for older people diagnosed with, or at risk of, complex health conditions and/or cognitive decline; and

3. Evaluate the outcomes for nursing care of older people.

For this task you need to prepare a 2500 word written assignment in a case study format. There are two parts to this assignment. Part 1 is a case study(750 words) and Part 2 is a plan of care for the person (1750 words).

Part 1 – Case Study (750 words)

· Introduce the purpose of the case study.

· Briefly outline the main problems as you recall and reflect on the care of an older person (over 65 years or over 55 years if Australia’s First Peoples) who has at least two (2) co-morbidities which impact on their functional status in an acute care (hospital) setting.

· Write a case study based on this experience. In the case study:

· describe the impact that specific stereotypes of older people may have on the person, and

· identify a specific, and recent environmental change impacting this older person.

Part 2 – Care Plan (1750 words)

· Develop a plan of care for the person. In your plan you must:

· Identify and justify the top five objectives for nursing care,

· Describe and justify evidenced-based nursing interventions to achieve each objective, and

· Create and describe the evaluation criteria that you will use to judge whether the plan addressed the identified objectives.

Throughout your care plan it must be clear how your decisions have been informed by a critical analysis of the case, applying concepts (stereotypes, nursing requirements, outcomes) raised in modules 1 and 2. No fewer than 10 different sources from the scholarly literature should be sourced. NOTE: It is important that you maintain confidentiality as per University guidelines and include no identifying demographic information about patients, colleagues or institutions.

Mobile Population and Sexual Health in China

Recently China has been facing mass migration of people within the country and people from other countries moving to China due to rapid industrialization which the country has realized. China has been rapidly constructing industries attracting investors as well as workers who want to work in the various firms. However, due to the large population, sexual health among the mobile population has brought concerns to the health sector and the public in general.

Many migrants lacked information about a sexual and reproductive health issue. According to (Zheng et al., 2001), about 65% had heard about the existence of STIs diseases but didn’t know the numerous examples of the STIs. Approximately 75% had not used contraceptive methods in their sexual lives. In the survey, about 14% of those who had sex in the month of the survey had used condoms (Zheng, et al., 2001). Factors which have promoted such worrying statistics among the migrants in China are attributed to the cultural, social and ethical practices of the mobile population.

Cultural factors that have led to lack of sexual health awareness involve the beliefs and values people consider as important to their sexual lives. Rampant sexual activities in China have associated with beliefs that sexual activities enhance emotional, psychological and physical health (Zheng, et al., 2001). Many people believe that low sexual activity was a risk factor for prostate cancer in men and breast cancer in women. The increased sexual activity could raise blood flow in the penis, and decrease the potential of acquiring hypertension, cardiovascular events, better homeostasis and even higher quality of life (Lin et al., 2005). The use of sexual acts as a source of physical pleasure and source of joy has boosted sexual activity among the China population and has tremendously interfered with sexual health as recommended by the health sectors.

Social factors form the majority factors which promotes rampant sexual activities among migrants. Taking into consideration the fact that the mobile populations are mostly new people in an area, the change of environment and interaction with newer people leads to the formation of a new relationship among the population. The education background of such people is of low level thus most lack knowledge on the use of condoms, drugs or other protective measures to mitigate risks of unhealthy sex (Choi et al., 2004). Age is also another factor which has proved a determinant of rampant sexual activities whereby the young adults leads (Zheng, et al., 2001). Young adults are more sexually active than aging population. The other factor involves the fact that as the migrants are moving, they are idle as they look for jobs so they have more time to look for partners and practice sex (Choi et al., 2004). Demographics and the varying races and ethnicity promote sex among the mobile population since they come from various backgrounds and once they reach urban centers, no one controls them, therefore migration acts as a source of freedom to the migrants (Zheng, et al., 2001). Another factor to be considered is the low standards of living among the mobile population, most of the migrants are doing manual jobs and unskilled jobs so they cannot afford health care services (Choi et al., 2004). Some are unable to pay for the drugs.

Ethical factors include lack of focus and misuse of offices by the government officials who are concerned with promoting sexual health among China’s population (Lin et al., 2005). Some Lack of moral values since some are married yet they look for partners in urban centers.

Recommendations

· The concerned sectors to run a campaign awareness to educate people on the sexual health and its importance

· Provision of better health care and free services in public hospitals so that lack of finance should not be an excuse

· Guide and counseling of young adults on the importance of faithfulness

Potential barriers to effective implementation of the recommendations include:

· Lack of enough funds to run the campaign and educate people

· Non- cooperative government

· Language barrier especially to the non-educated population

You will become the theorist immersing yourself in the writings from the earliest mentioned to the most current.

As you read, look at how you (taking on the persona of the theorist) developed the theory.

In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed.

In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring.

The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care.
