Assessment of Cardiac Status

Esther Jackson is a 56-year-old black female who is 1-day post-op following a left radical mastectomy. During morning rounds, the off-going nurse shares with you during bedside report that the patient has been experiencing increased discomfort in her back throughout the night and has required frequent help with repositioning. She states that the patient was medicated for pain approximately 2 hours ago but is voicing little relief and states that you might want to mention that to the doctor when he rounds later this morning. With the patient appearing to be in no visible distress, you proceed on to the next patient’s room for report.Approximately 1 hour later, you return to Ms. Jackson’s room with her morning pills and find her slumped over the bedside stand in tears. The patient states, “I don’t know what is wrong, I don’t feel right. My back hurts and I’m just so tired. What is wrong with me?” The patient refuses to take her medications at this time stating that she is starting to feel sick to her stomach.Just then the nursing assistant comes into the patient’s room to record Ms. Jackson’s vital signs, you take this opportunity to quickly research the patient’s medication record to determine if she has a medication ordered for nausea. Upon return, the nursing assistant hands you the following vital signs: T 37, R 18, and BP 132/54, but states she couldn’t get the patient’s pulse because “it is all over the place.”Please address the following questions related to the scenario. What do you suspect is the cause of the patient’s symptoms?Describe the course of action that you will take to confirm this suspicion and prevent further decline. What further assessments, lab values, and tests will likely be ordered for this patient and how often? If testing is to be completed more than once, please explain the rationale for doing so.While you are caring for this patient, how will you ensure that the needs of your other patients are being met?

Discussion Forum #7 Family Systems TheoryPublic health nurses must blend family nursing theories with public health theories and frameworks to work both with individual families and populations of families. Explain the following three family social science theories, including the strengths and weakness of each one: Family Systems Theory, Family Developmental and Life Cycle Theory and Bioecological Systems Theory. Which theory is most beneficial to you as a public health nurse in achieving healthy outcomes for families and why?

Essentials of Evidence-Based Practice

June responseDeveloping a PICOT question takes time and thought. A major component in this process includes understanding how to create the PICOT question. The question should consist of population, intervention or issue of interest, comparison, and the timeframe to accomplish the outcome (Melnyk & Fineout-Overholt, 2019, p.34)My topic of interest is Transcranial Magnetic Stimulation and depression. Transcranial Magnetic Stimulation Therapy (TMS) is a non-invasive treatment that uses a magnetic coil placed on the left dorsal lateral prefrontal cortex to stimulate nerve cells in the brain to improve symptoms of depression. TMS is an alternative treatment that is used when antidepressants have not been effective (Mayo Clinic, 2018). My PICOT question is ‘What is the effect of Transcranial Magnetic Stimulation Therapy for Adults with Depression as Reflected in Depression Rating Scales?’I used CINAHL Plus, Thoreau, and APA PsycInfo to conduct my search. The words I used initially to search were TMS and depression. Then, I broadened my search by adding rating scales, depression rating scales, depression assessment tools, and adults. I also used the search guides of peer-reviewed and stipulated the years in the sidebar to range from 2016 to 2022. At the top of the EBSCOhost page, directly above the search bar where one can type in the qualifiers, there is an option to choose databases (Walden University Library, 2022).  One has the option to search several different databases at the same time. I was able to find randomized clinical trials, a pilot investigation, a cohort study, and two double-blind, randomized, parallel-group, sham-controlled clinical trials.Understanding how to conduct research is imperative to all nurses. It is essential to understand how to conduct research and vital to have the capacity to interpret the research. Whenever a nurse presents an issue to be addressed to their supervisor or any key stakeholder, it should always be accompanied by evidence-based research.ReferencesMayo Clinic. (2018). Retrieved March 21,2022, from https://academicguides.waldenu.edu/az?s=19981Colton responseThe clinical issue I have selected is safety in inpatient psychiatric facilities. When providing care to individuals with mental health disorders, aggression and violence can be a common occurrence in psychiatric facilities, especially when patients are admitted involuntarily. This patient population may act in an aggressive or violent manner related to altered reality, behavioral disorders, manic phases, attention seeking behavior, etc. (APA, 2021). To counter these behaviors and increase safety for the patient, peers, and staff members, de-escalation techniques should be emphasized, utilized, and improved in an attempt to avoid physical intervention to increase safety.I used multiple databases to conduct my search for research articles relevant to my clinical issue. I used CINAHL Plus, PubMed, and BioMed Central (Walden, 2021). I began my search by using the term “de-escalation” and narrowed the results down by adding terms like “inpatient, aggression, mental illness, and restraints.” As I added and removed these terms the databases would refine the search and allow me to review various articles related to my clinical issue. The ability to constrict the search terms down to the year 2017-2022 makes finding up-to-date information very convenient. Using the key terms to only find articles relevant to my topic saved me a lot of time.Understanding what strategies help to build a PICOT question is very helpful to efficiently forming an appropriate question. Strategies like brainstorming terms, using synonyms and similarities, and developing a list of different combinations of key terms to modify the search (Ho et al, 2017). My list of terms included seclusion, de-escalation, violence, aggression, mental illness, inpatient, involuntary, injuries, safety, etc.References:American Psychological Association (APA) (2021). Each week students should also explain how they met a course competency or course objective(s).In each week’s entry, students should reflect on the personal knowledge and skills gained throughout the course. Journal entries should address one or more of the areas stated below.  In the Topic 10 graded submission, each of the areas below should be addressed as part of the summary submission.New practice approachesInterprofessional collaborationHealth care delivery and clinical systemsEthical considerations in health carePractices of culturally sensitive careEnsuring the integrity of human dignity in the care of all patientsPopulation health concernsThe role of technology in improving health care outcomesHealth policyLeadership and economic modelsHealth disparitiesWhile APA style is not required for the body of this assignment, solid academic writing is expected, and in-text citations and references should be presented using APA documentation guidelines, which can be found in the APA Style Guide, located in the Student Success Center. This assignment uses a rubric. 1. Minimizing bias and decreasing threats to internal validity is important to experimental designs. How will a researcher use the three criteria, manipulation, randomization, and control to minimize bias and decrease threats to internal validity?2. Many times, researchers state that randomized clinical trials (RCT) provide the strongest level of evidence for an individual study when using an evidence-based model.  As a researcher, why do you think this statement is true?3. When conducting an experimental design, how will you as a researcher, use intervention fidelity to increase the strength and quality of the evidence provided by the findings of the study?4. What is your cosmic question? (This is a question you ask your peers to respond to based on the chapter discussed in class this week i.e. Quantitative studies).Resourcehttps://www.youtube.com/watch?v=10nMNh3RMp0&t=3s
