Addressing Chronic Disease
According to the Population Health course text, “Roughly 40 million Americans are still uninsured and 112 million Americans (almost half of the U.S. population, 45%) suffer from at least one chronic condition” in the United States, an estimated 125 million persons have at least one chronic condition, and half of these persons have multiple chronic conditions” (Fabius, and Pracilio, Nash, Clark, 2015, p. 4 ).

This week’s Learning Resources examine numerous health problems that result in a need for ongoing care. As you have explored this week, many costs are associated with chronic disease—both in terms of lives lost and socioeconomic burden. What can be done to help reduce chronic disease at the population level?

For this Discussion, you will take an in-depth look at chronic disease, and you will evaluate ways to address this issue through the application of chronic disease models and frameworks. In addition, you will consider the impact of the challenges of managing chronic disease on quality of care delivery.

To prepare:

Review the application of chronic disease models as a method for managing chronic diseases at the population level.
Consider characteristics of chronic disease models and how to apply them as presented in the Learning Resources.
Consult Figure 13–7 (p. 267) in Population Health: Creating a Culture of Wellness and consider examples of determinants and outcomes of population health with chronic diseases in a specific subpopulation. Then, select one chronic disease on which to focus for this Discussion.
Ask yourself, “What are the challenges of managing this chronic disease? How do these challenges limit the ability to deliver effective quality care?” Conduct additional research using the Walden Library and credible websites as necessary.

Case study

Memorial Hospital is a privately owned 600-bed facility. The GeneralHospital, located only a few miles from Memorial, is affiliated with the local School ofMedicine and offers up-to-date services in those specialized areas. Instead of tryingto compete with General Hospital to provide special services, Memorial Hospital hasconcentrated on offering high-quality general health care at an affordable price.Compared with the much larger General Hospital, Memorial stresses close personalattention to each patient from a nursing staff that cares about its work. In fact, thehospital has begun to place ads in newspapers and on television, stressing itspatient-oriented care.

However, the hospital’s administrator, Janice Fry, is concerned about whether thehospital can really deliver on its promises, and worries that failure to provide thelevel of health care patients expect could drive patients away. Janice met recentlywith the hospital’s managerial personnel to discuss her concerns. The meetingraised some questions about how the hospital’s quality of health care could beassured. Jessica Tu, director of nursing, raised the question, “How do we measurethe quality of health care? Do we give patients a questionnaire when they leave,asking if they were happy here? That does not seem to answer the questionbecause we could make a patient happy, but give them lousy health care.” Severalother questions were asked concerning the hospital’s efforts to keep costs down.Some people were concerned that an emphasis on costs would be detrimental toquality. They argued that when a person’s life is at stake, costs should not be ofconcern.

After the meeting, Janice began thinking about these questions. She rememberedreading recently that some companies were using total quality management (TQM)to improve their quality. She liked the idea—if it could be used in a hospital.

This is obviously a very tragic case on many levels.
1.) What does this case tell you about the power of biology?
2.) Does it support or refute Dr. Money’s (and others’) apparent view that children are a “blank slate,” and that they can successfully transition from one sex to another if it is done early enough in childhood?

Introduction
This lesson will cover peer interactions, friendship and gender. We will begin by exploring how peer interactions develop, and what peer acceptance means in the context of child development. We will investigate how social information-processing functions in popular and unpopular children, and the impact of this on children’s short- and long-term development. We will then move onto friendship, where we will look at what friendship means in the different stages of development. We will look at how groups function, and the importance of teenage romances. We will also look at gender, and the impact of gender stereotypes in child development. We will discuss the role of biology, cognition, family and other social influences on the genders. Lastly, we will have a brief discussion about sexual orientation and androgyny.
