The amount if big data can be difficult for those required to examine and retrieve information. Despite the overwhelming amounts of big data, it does come with benefits, challenges and risk associated with its use.A benefit to using big data is the ability to communicate with a large group of patients at one time. For instance, a physician can send a message out to a large group of patients in a short period of time with the use of big data (Laurete Education, 2018).  A physician’s office can remind large group of patients of appointment reminders, lab appointments or alert them to call the md office with abnormal lab results.  This cost effective and convenient as it allows consistent measures to monitor the practice.A potential challenge of using big data is the fine line of deciding who owns and who can use the data (Shanthagiri, 2014). For example, the data sources include the patient’s private health information which could now be potentially shared with labs, pharmacies, social media in addition to their physician (Shanthagiri, 2014).  The enforcement of privacy of PHI and HIPPA is strict. The potential for error with big data is a high risk.Working with the VA medical center, I observe strict monitoring and firewalls ensuring safety of patient’s PHI. The vulnerability of exposure of personal health information to social media or social networks is a constant threat.  The EMR not only stores health data but also houses the patient’s personal information such as social security numbers, addresses, and birthdays. A strategy to combat this potential problem is a strong malware and security system monitored 24 hours a day 7 days a week. The VA medical center monitors all information going in and going out of the EMR with it’s own security department specific for data protection.  Each user is tagged to this information and can be traced.  The VA will immediately terminate privileges should a user of the EMR violate any firewall in the system. They monitor everything as they should.As a bedside nurse I am aware of my contribution to this data however, I am not exposed to the cumbersome job of disentangling this information or protecting it. I know the potential benefit of collecting this information is to provide information as it relates to quantifying and qualifying illness, providing evidence for practice.  That being said, big data will continue to grow as we knowledge workers continue to contribute and technology advances.ReferencesGephart, S., Davis, M., & Shea, K. Testicular Cancer Case Studies A 21-year-old male noted pain in his right testicle while studying hard for his midterm college examinations. On self-examination, he noted a “grape sized” mass in the right testicle. This finding was corroborated by his healthcare provider. This young man had a history of delayed descent of his right testicle until the age of 1 year old. Studies Results Routine laboratory studies Within normal limits (WNL) Ultrasound the testicle Solid mass, right testicle associated with calcifications HCG (human chorionic gonadotropin) 550mIU/mL (normal: <5) CT scan of the abdomen Enlarged retroperitoneal lymph nodes CT scan of the chest Multiple pulmonary nodules Diagnostic Analysis At semester break, this young man underwent right orchiectomy. Pathology was compatible with embryonal cell carcinoma. CT directed biopsy of the most prominent pulmonary nodule indicated embryonal cell carcinoma, compatible with metastatic testicular carcinoma. During a leave of absence from college, and after banking his sperm, this young man underwent aggressive chemotherapy. Repeat testing 12 weeks after chemotherapy showed complete resolution of the pulmonary nodules and enlarged retroperitoneal lymph nodes. Critical Thinking Questions 1. What impact did an undescended testicle have on this young man’s risk for developing testicular cancer? 2. What might be the side effects of cytotoxic chemotherapy? 3. Skill building and training in positive thinking and a focus on positive emotions assist nurses to better listen, encourage, motivate, and create connections (Huber, 2014). Having self awareness simply means that you understand you. You have an awareness of what makes you tick as well as what makes your heart sing or sink.  When you know your strengths and weaknesses as a person, you can then start to understand why you feel the way you do and what makes you feel the way you feel. When you can learn to understand your own emotions, as well as how to control them, you can identify their impact to you and those in your team (Candy, 2016).I have a personal example of this from a current situation that I am facing on my job in which I am the program manager. In the last two months I have had two of my staff to move from our area.  Recently, one of the two Nurses I have left has decided to transfer to another clinic to “work less days”. She is fully aware that this will only leave our program with one nurse other than myself. This nurse shared this information to me openly in front other team members (Charge nurse, Social worker, dietician, secretary). It was so quiet you could hear a pin drop. I politely said ok, I want you to do what is going to make you happy and work for your family. Now the other team members didn’t take this news so calmly. What I have learned through the years from trial and error is that as the Leader I cannot allow my feelings to control my behavior. Deep down inside I wanted to scream because it takes 6 months for a new nurse to take call and train patients and I’m in school. To this day my staff are still not aware of how I really feel about the situation. As the leader of our program I could not afford to display the heart drop in my heart that I felt deep down inside because I didn’t want to envoke uneccessary anxiety on them. Surprisingly they have come to privately stating how surprised they were that  I was able to remain so calm after hearing that another nurse was leaving. Of course after hearing that I could only say “Thank you Lord” because that is an area that I have really had to seek God’s help with.Candy, L (2016).
