The procedure needs to be researched if it is within the standards of practice. The standards of practice are the science of nursing with specific details associated with a specialty procedure (ANA,2016). Each specialty has its own standard of practice. This will help determine if the new procedure can be adapted. The procedure needs to be commonly recognized, standard of practice in the clinical area, and doesn’t require diagnosis or medical knowledge (CA-BRN, 2011). There will need to be a collaboration with other facilities within that state and then nationally to determine if this new procedure is an acceptable practice for that area. The procedure will need to be an evidence-based practice.Once this new procedure is a viable change, it will then need to be put into practice. The staff will need to be educated on this new procedure and documentation through in-services, literature information and skills training. Applicable information will be provided to all persons involved including physicians, nurses, and ancillary staff. Examples include foreign bodies such as calculi (stones), indwelling catheters or other drainage devices, obstruction, immunosuppression, renal failure, renal transplantation and pregnancy. UTI in the elderly is almost always complicated in men with prostatic hypertrophy and in post-menopausal women who may have an increased post-void residual volume.A urinary tract infection is an infection anywhere in the urinary tract. These often cause inflammation of the urethra (urethritis), bladder (cystitis), or kidneys (pyelonephritis).The symptoms depend on the area affected. Typical symptoms of a lower UTI include frequency and dysuria without fever, chills or back pain whereas upper UTI usually presents with symptoms of pyelonephritis such as loin pain, flank tenderness, fever or other signs of a systemic inflammatory response.Urinary tract infection is diagnosed by examination of a urine culture. Further investigations using intravenous urography or ultrasound scanning may be necessary. Most urinary tract infections are treated with antibiotic drugs. Increasing fluid intake and taking preparations such as potassium citrate that make the urine less acidic can relieve the symptoms.Trimethoprim/ sulfamethoxazole (Bactrim)Antibiotics—trimethoprim/ sulfamethoxazole (Bactrim) remains the first choice for community-acquired UTI in most areas. Complicated UTI is caused by a wider spectrum of organisms, and recommendations for treatment differ. Guidelines on specific antibiotic treatment and duration of treatment are available, but with increasing antibiotic resistance (including of E. coli to Trimethoprim), local microbiological advice should be taken into account when choosing antibiotic treatment.Advantages: Cheap, Well tolerated, high concentrations in vaginal and peri-urethral fluid. Inadequate preoperative patient preparation leading to cancellation of the surgery is a stressful experience to the patient and the family not forgetting the wastage of resources planned for the activity which is a loss to the organization. Cancellation of elective scheduled operations on the day of surgery leads to an inefficient use of operating room (OR) time and a waste of resources. It also causes inconvenience for patients and families. (Kaddoum, et al. 2016). Factors that lead to elective surgical cases being cancelled on the day of surgery include laboratory investigation values that are out of required range, not observing the starving period, poor communication on time of surgery, incomplete medical evaluation, or lack of financial clearance. Quality improvement strategy that can be applied to address the problem would be developing a standard operating procedure where the preoperative preparations for the patient are reviewed 24 hours before surgery and documentation of the communication to the patient, surgeon, anesthesia care provider and the OR manager is done before confirming the scheduling.Continuous quality improvement (CQI) is the responsibility of all nurses and is vital when addressing the challenges of the health care industry.
