Determinants of Health

What are some factors that make some people healthy and others ill? Healthy People 2020 identifies five determinants of health that influence the health of individuals and populations. Healthy People 2020 describes them as “a range of personal, social, economic, and environmental factors that influence health status” (U.S. Department of Health and Human Services, 2014, About Determinants of Health, para. 1). Determinants fall into five categories including: (a) policy making, (b) social factors, (c) health services, (d) individual behavior, and (e) biology and genetics.

Go to the “Determinants of Health” link at http://www.healthypeople.gov/2020/about/foundation-health-measures/Determinants-of-Health (Links to an external site.)Links to an external site. and click on each determinant to read about them.

Choose one of the five determinants. In your post, describe this determinant and its importance.

Discuss how this determinant relates to one intervention from the Public Health Intervention Wheel (Nies & McEwen, 2015, pp. 14–15).

Demographic and Epidemiological Assessment

For this discussion, you will collect assessment data about your city or county. This post will include information about demographics (general characteristics) and epidemiological data (disease or health behavior rates) of your community.

Demographic data: Go online to the U.S. Census Bureau at https://www.census.gov/quickfacts/ (Links to an external site.)Links to an external site.. Obtain a range information about the demographic characteristics of the population for your city or county of residence. You may have to look at county data if your city is not listed. Discuss demographic data about age, ethnicity, poverty levels, housing, and education.

Epidemiological data: Go to your city or county health department website (search the Internet) or County Health Rankings (http://www.countyhealthrankings.org/ (Links to an external site.)Links to an external site.) and report epidemiological data about your area.

Identify several priority health concerns for your area.

The Instructions on Finding Demographic Data on the U.S. Census Website (Links to an external site.)Links to an external site. will assist you in using this website as a resource.

Caring for Vulnerable Populations

Review the Week 3 Case Study (Links to an external site.)Links to an external site.. This case study focuses on a vulnerable young woman who is facing many risk factors.

Discuss several risk factors that may impact health outcomes for vulnerable populations.

Assurance is one of the three core functions of public health. Find a resource in your community (other than WIC) that could assist Mary during her pregnancy. Start by searching the Internet for your local health department’s website. What services does it provide for pregnant women, those living in poverty, or those with mental health concerns? What about your local welfare office? Does it provide prenatal or mental health services? Are there any service organizations, crisis pregnancy centers, or churches providing help for pregnant women or those experiencing mental health concerns? (The Internet, your local newspaper, and the telephone book may also help you locate resources.)

Briefly describe the types of services that you found.

Choose one agency and assess this agency in terms of the 4 A’s. Is it accessible, acceptable, affordable, or available for Mary or other pregnant women that you case manage?

Are there adequate resources for other vulnerable populations in your community?

Length: The completed form should be three to four pages in length (not including any optional references).

Guidelines

Below are the requirements needed for successful completion of the Windshield Survey Form.

Introduction to the Community: Identify the community you will be using for this assignment with the city and state and provide a brief, one paragraph description of the community. Your community should be the area where you live or the area surrounding your work setting. The community must include a residential area and be broad enough to answer the survey questions. You do not need to include demographic data.

Windshield Survey: Assess your community by doing a windshield survey. Information about the components of a windshield survey is located in your textbook, Nies & McEwen (2015) on page 98 (Box 6-2). Drive through the area and report your observations by describing each of the following six areas using a majority of the questions from each category of Box 6-2 as your guide (one to two in depth paragraphs for each category). Be sure to include what you observed related to each of these categories, and also include any significant items that are missing in your community because this may be equally important in identifying a community health problem.

Community vitality

Indicators of social and economic conditions

Health resources

Environmental conditions related to health

Social functioning

Attitude toward healthcare

Note: It is helpful to conduct this assessment at least two different times: during the day or evening, on a weekday and/or on the weekend. If possible, plan on asking someone to drive during your survey so that you can take notes.

Vulnerable population: Identify an aggregate or vulnerable population in your community based on your observations. What did you observe about this population? See Units five (5) and six (6) in Nies and McEwen (2015) for examples of vulnerable populations and aggregates.

Conclusion: Provide a summary of your findings. Describe several potential community health problems for the vulnerable population that you identified.

References: The purpose of this assignment is to document your observations of your community. Outside sources are not required. However, if sources are used, these sources must be cited within the form (Author, year) and listed in the reference section in APA format (see APA resources for help with formatting).

Purpose

The purpose of this PowerPoint presentation is to provide an opportunity to develop a community health nursing intervention and evaluation plan for a community health problem that you identified in your community (described in Milestone 2: Vulnerable Population Assessment). You will apply the components of the nursing process to assist this vulnerable population and develop a proposal that could be presented to community leaders.

Course Outcomes

This assignment enables the student to meet the following course outcomes.

CO1: Apply principles of nursing theory to the public health system by analyzing determinants of health and the public health intervention wheel. (PO 1)

CO3: Plan prevention and population-focused interventions for vulnerable populations using professional clinical judgment and evidence-based practice. (PO 4, 8)

CO4: Evaluate the delivery of care for individuals, families, aggregates, and communities based on theories and principles of nursing and related disciplines. (PO 1)

Due Date

Submit your PowerPoint presentation by 11:59 p.m. MT Sunday by the end of Week 6.

Points

This assignment is worth a total of 225 points.

Scenario

You are a community public health nurse (C/PHN) working with the vulnerable population you identified in Milestone 2. You will choose a specific community health role that would serve this population. You have analyzed the data collected from your windshield survey and vulnerable population assignments (the first two milestones) and identified one community health problem that a nurse can impact. Now, review the scholarly literature or scholarly websites listed below and identify one specific evidence-based nursing intervention that has the potential to improve the health of this group. Then develop a proposal to implement this intervention and a plan to evaluate the outcomes of this intervention. Create a PowerPoint that you could use to present to an organization in your community to request approval for funding and support for this intervention. (You are not required to implement the intervention or present your PowerPoint, though you may want to consider implementing your project in the future).

First response
While researching the wrong-site surgery, I stumbled across and article that has an interesting approach to helping prevent wrong-site surgeries. A checklist, it seems obnoxiously simple, but the fact remains in the study conducted by over 10 doctors that out of 133 qualifying cases, 28 of these wrong-site cases would have been prevented using a checklist. I’m not going to read the whole study to show how they got the results, but this is based on facts. Just using something as simple as a follow along checklist can make a huge difference.
Think about the mistake on the nurse’s part, she must have seen the surgery marks on the patient’s right knee where the operation should occur…but because she THOUGHT it was the wrong knee, she just switched the surgery site. That is absurd; a checklist would have a step to ensure that everybody having anything to do with the operation knows exactly what is being operated on. There should be no questions at all about that.
I’m in the Air Force and we are required to use checklists for many of our maintenance habits and pilots must follow them in order to fly a jet. No matter how many years they have been a pilot they must follow the same checklist every single flight. And this is done for a reason…research proves they lessen the likelihood of mistakes.
The following is the average length of stay (days) per condition in 2012
Abdominal pain = 2.836 Days
Acute myocardial infarction = 4.593 Days
Chronic obstructive pulmonary disease and bronchiectasis = 4.279 Days
Diabetes mellitus with complications = 4.602 Days
The following is the average length of stay (days) and average hospital costs in 2012 for patients with the following procedures:
Cesarean section = 2.728 Days $17,340 Average Cost
Hip Replacement, total and partial = 3.849 Days $58,326
Hysterectomy = 2.433 Days $33,180
Percutaneous coronary angioplasty = 3.232 Days $70,083

Learning Outcomes

After reading this article, you will be able to:

  • Discuss the role of genetics in drug addiction. • Identify the contribution of self-control to drug addiction.

The human brain may be “wired up” for addictive behav- iour according to a study that shows how some people are more likely than others to become addicted to crack cocaine.

Scientists have found specific abnormalities in the brains of regular cocaine users which are likely to have been present in early childhood rather than coming about as a result of the drug misuse.

The researchers also found similar abnormalities in the brothers and sisters of cocaine addicts—even though the sib- lings were not themselves drug users—but did not find the same brain patterns in the general population.

The discovery of specific brain abnormalities in the families of drug addicts suggests a genetic basis for addictive behav- iour. But it also implies that some people can overcome this predisposition to remain free of drugs, said Karen Ersche of the University of Cambridge.

“Cocaine is a highly addictive drug but only some people get hooked on it. However, your chances of getting hooked rise about eight times if you have a family member who is addicted,” Dr Ersche said.

“Our findings suggest that drug addiction is not a failure of character or a life-style choice. It’s a problem with the brain.

If your brain is wired for addiction it’s easier for the drugs to take over, but the good thing is that this is not inevitable,” she said.

The study, funded by the Medical Research Council and published in the journal Science, used hospital scanners to anal- yse the brains of 50 cocaine addicts and compared them against the brain scans of their nonaddicted siblings. As an overall con- trol, the researchers also scanned the brains of 50 unrelated, healthy volunteers.

The scans showed that both the addicts and their siblings shared defects in the nerve fibres that communicate with the front part of the brain, the temporal cortex, which is known to be involved in controlling impulsive behaviour.

Previous research has shown that drug addiction is linked with brain abnormalities involved with self control but it was not known whether the drug misuse was the cause or the result of the irregularities in the brain.

“Given that some forms of drug addiction are thought to develop out of bad habits that get out of control, it’s intriguing that siblings who don’t abuse drugs show similar brain abnor- malities as the ones who have been abusing drugs for many years,” Dr Ersche said.

“Our findings now shed light on why the risk of becoming addicted to drugs is increased in people with a family history of drug or alcohol dependence—parts of their brains underlying self-control abilities work less efficiently,” she said.

“The use of drugs such as cocaine further exacerbates this problem, paving the way for addiction to develop from occa- sional use,” she added.

The next stage of the research is to find out why the drug- free siblings were able to avoid getting hooked on drugs.

Drug Addiction May Be Hereditary, Study Suggests by Steve Connor

They may have developed other interests that led them away from drug taking, or have been influenced by older family members.

“While we still have more work to do to fully address the reasons why some family members show a greater resilience against addiction, our results will provide the scientific basis for the development of more effective prevention and treatment for people at risk,” Dr Ersche said.

Professor Les Iversen of the University of Oxford said: “These new findings reinforce the view that the propensity to addiction is dependent on inherited differences in brain cir- cuitry, and offer the possibility of new ways of treating high- risk individuals to develop better ‘self control’.”

Can descriptive statistical processes be used in determining relationships, differences, or effects in your research question and testable null hypothesis?

Hypothesis might be ” There will be decreased work reported difficulties with decreased relational abilities of forensic psychologists in correctional environments.”
Null: There will be no differences in work related difficulties in forensic psychologist with decreased relational abilities in correctional environments. ”
This way we can have data to measure, i.e. reported work related problems and relational abilities, probably via a scale.

Re-Educating Health Care Providers on Hand Hygiene Practice”

Re-Educating Health Care Providers on Hand Hygiene Practice”

Hospital Acquired infections (HAIs) present a serious challenge in provision of healthcare services to patients in hospital settings. Studies indicate that about 80 percent of patients who contract HAIs often succumb to the diseases and die while receiving treatment in the hospital (Ivers et al. 2012). The numbers have since risen up and it has become increasingly difficult for researchers to point to the exact numbers of patients suffering from HAIs. Nevertheless existing literature has produced a lot of evidence pointing to the role of hand hygiene practices in reducing the rates of HAIs. However compliance rates to hand hygiene practices such as hand washing and gelling remain low which makes it difficult to prevent HAIs. There is also a gap in available literature concerning the impact of specific hand hygiene programs in reducing the rates of HAIs. This project assesses how reeducation can enhance compliance to hand hygiene in hospital settings thereby reducing the rates of HAIs. Hand hygiene encompasses various technics and practices for cleansing hands before and after treating patients. Several agents such as antiseptic agents anti-microbial soaps alcohol based rubs plain soaps and antiseptic agents play a critical role in hand hygiene practices.
The main underlying factor is that compliance with hand hygiene practices reduces the rates of related infections to a greater extent (Ivers et al. 2012). However inadequacies exist in the literature available regarding how to increase compliance with hand hygiene practices before and after attending to each patient according to the guidelines stipulated by the WHO (Dennison & Prevost 2012). …Show more “

DISCUSSING THE GENETIC OR PHYSIOLOGICAL EVIDENCE THAT SUPPORTS THE NOTION THAT BIOLOGY PLAYED A KEY ROLE IN EXPLAINING THE OFFENDER’S CRIMINALITY.

Choose a criminal offender who committed crimes due to a biological reason. Write a 1,400- to 2,100-word paper discussing the genetic or physiological evidence that supports the notion that biology played a key role in explaining the offender’s criminality. Research the behaviors that constitute psychopathy. Discuss in detail the specific behaviors demonstrated by the offender that align with behaviors indicative of a psychopathic individual. Case examples include the following: Andrea Yates and the documented evidence of psychiatric issues, including post-partum depression, prior to the murders of her five children Charles Whitman, known as the Texas Tower Sniper, and the presence of a tumor discovered post-mortem alleged to have played a role in his overt acts of aggression John Hinckley, Jr. and the documented evidence of psychiatric issues leading to his controversial insanity defense in the shooting of President Ronald Reagan Include at least two academic references.

What does this tell us about contemporary American/Christian culture?  Why is this important to know?

Critical engagement of Schmidt, chapters 1-3
Find one idea, concept, point, piece of text, etc., that you found interesting (in a good way or a bad way—it could be something you agreed with or something you disagreed with) in these chapters.  Then reflect on it.  Don’t just summarize what the text says.  Think about what the concept (idea, etc.) means to you.  Why does this particular idea stand out to you?  Critically engage the point—I don’t want you to just tell me the point, summarize the point, or describe the point.  Tell me why this point or idea or passage that you chose is significant.  This requires some thinking.  Think about these kind of things: whatever it is that the author is saying, what are the consequences of accepting it as true?  What are the consequences of rejecting it?  What does this tell us about contemporary American/Christian culture?  Why is this important to know? You don’t need to address any or all of these questions, but I want you to think about why a certain part of this text would be significant. I want most of your paper to be your voice telling me the significance of some part of the text that caught your attention or made you think.  Your paper should NOT include large quotations or summary of the text.
