Caring for Populations Milestone 3:

Intervention and Evaluation PowerPoint

Guidelines and Grading Rubric

Purpose

The purpose of this PowerPoint presentation is to provide an opportunity to develop a community health nursing intervention and evaluation plan for a community health problem that you identified in your community (described in Milestone 2: Vulnerable Population Assessment). You will apply the components of the nursing process to assist this vulnerable population and develop a proposal that could be presented to community leaders.

Course Outcomes

This assignment enables the student to meet the following course outcomes.

CO1: Apply principles of nursing theory to the public health system by analyzing determinants of health and the public health intervention wheel. (PO 1)

CO3: Plan prevention and population-focused interventions for vulnerable populations using professional clinical judgment and evidence-based practice. (PO 4, 8)

CO4: Evaluate the delivery of care for individuals, families, aggregates, and communities based on theories and principles of nursing and related disciplines. (PO 1)

Due Date

Submit your assignment to the appropriate basket in the Dropbox by 11:59 p.m. MT Sunday of Week 6.

Points

This assignment is worth a total of 225 points.

Scenario

You are a community public health nurse (C/PHN) working with the vulnerable population you identified in Milestone 2. You will choose a specific community health role that would serve this population. You have analyzed the data collected from your windshield survey and vulnerable population assignments (the first two milestones) and identified one community health problem that a nurse can impact. Now, review the scholarly literature or scholarly websites listed below and identify one specific evidence-based nursing intervention that has the potential to improve the health of this group. Then develop a proposal to implement this intervention and a plan to evaluate the outcomes of this intervention. Create a PowerPoint that you could use to present to an organization in your community to request approval for funding and support for this intervention. (You are not required to implement the intervention or present your PowerPoint, though you may want to consider implementing your project in the future).

Describe the identified problem

Include at least two important findings that demonstrate that this is a problem in your community.

Describe your specific community health role: Some examples of roles are school nurse, parish nurse, home health nurse, occupational nurse, health department nurse etc.

Identify the purpose of the presentation which is related to proposing a community health intervention and evaluation plan.

Identify an evidence based intervention: (average of 2–3 slides).

Find an evidence-based nursing intervention from a peer-reviewed journal article OR one of the following databases. These databases evaluate research evidence about community health interventions. Be sure that your intervention is one that is recommended. Cite your source on the slide with the author or organization and year.

Use at least one of the following to identify an evidence based intervention:

A peer reviewed journal article demonstrating that your intervention has been successful

The Community Guide (http://www.thecommunityguide.org/) Choose one of the topics in the topic tab to find interventions.

The CDC Community Health Improvement Navigator Database of Interventions (http://wwwn.cdc.gov/chidatabase)

Healthy People 2020 evidence-based interventions. Find a relevant topic area at (http://www.healthypeople.gov/2020/topics-objectives), click on that topic area, and then click on the “Interventions and Resources” tab to see the evidence-based interventions list for that topic area

Provide a brief overview of your intervention.

Discuss why this intervention is a good fit for your community (why you expect it to be effective in your community).

Intervention Implementation: (average of 3–4 slides).

Describe how you would implement your community health nursing intervention in your community

Discuss the actions will you take as the community health nurse to accomplish this intervention.

Relate your actions to the Public Health Intervention Wheel (Nies & McEwen, 2015, p. 14, Figure 1-3)

Identify your target population.

Describe how you will reach out to your target population.

Include strategies to engage your target population.

Describe where the intervention will take place.

Identify when your intervention will take place

Will it take place once or multiple times?

Identify those in the community that you will collaborate with (e.g., physician’s office, church, local resources, etc.).

2

Chamberlain College of NursingNR443 Community Health Nursing

Explain what level(s) of prevention is your intervention addressing (primary, secondary, or tertiary prevention)?

Proposed Evaluation Methods: (average of 2–3 slides)

Your presentation must include at least one proposed quantitative or qualitative evaluation method that you would use to determine whether your intervention is effective. Outcome measurement is a crucial piece when implementing interventions.

Describe the method you would use to evaluate whether your intervention was effective. (There is a helpful tool found in Course Resources to assist you with understanding qualitative and quantitative methods of evaluation).

Identify the outcomes you would track to show whether your intervention works (eg. BMI would be one outcome you could track for a weight loss intervention).

When would you measure these outcomes?

Describe the short-term and long-term impact on your community if the intervention is successful.

Summary: (average 1 slide)

The summary should reiterate the main points of the presentation and conclude with what you are asking to be accomplished; for example, “Based on ABC, it is imperative our community has XYZ. Can we count on your support? Thank you for your consideration.”

In addition to the slides described above, your presentation should include a title slide with your name and role and a reference slide. Cite all sources used with (Author, year) on the PowerPoint slide.

The slides should include the most important points in short bullet pointed phrases. Do not include full sentences or paragraphs on the PowerPoint slides. You may add additional comments in the notes section to clarify information for your instructor.

The slides should be visually appealing and not overcrowded. Use an interesting template and include some clipart or graphics for interest.

Reflect on your hospital setting and the use of      evidence-based practice models.

New Models for Evidence-Based Practice
A professional goal for DNP-prepared nurses is to produce new evidence-based models of care and develop evidence-based guidelines. As you continue to develop and engage in your EBP Project, keep this aim in mind. What could help to facilitate the acceptance of new practices?
As noted in earlier weeks, every specialty area should base practice on research and evidence. For example, in informatics, an informatician who plans to implement a new health information system would be wise to follow a model that research shows has been effective. Similarly, those involved in leadership or management can review change models and theories, exploring current evidence on best practices that produce positive results. For this Discussion, you evaluate evidence-based practice models, change theories, and frameworks that assist with the adoption of new evidence-based practice approaches.
To prepare:

  • Reflect on your hospital setting and the use of      evidence-based practice models. How are these models used? If they are not      used, how might the integration of an evidence-based practice model change      practice?
  • Review the Rosswurm and Larrabee article      presented in the Learning Resources focusing on the model they present for      integrating new evidence-based practices (See  attached file).
  • Research and locate an evidence-based practice      model from nursing or another discipline that could be used in your      specialty area to lead a systematic change to evidence-based practice      approaches. You may also use a model or framework discussed in your course      text, Translation of Evidence into Nursing and Health Care      Practice.
  • Consider how using a model for evidence-based      practice can facilitate adoption of new approaches to practice.

By tomorrow Tuesday 9/25/18 10 pm, write an essay of a minimum of 550 words in APA format, and at least 3 scholarly references from the list of required readings below. Include all level one headings as numbered below:
Post a cohesive response that addresses the following:
1) Discuss the use of evidence-based models in your hospital setting.
2) Describe a model for evidence-based practice or change theories and frameworks that you could utilize to facilitate the adoption of new approaches to practice. Substantiate your selection with evidence from the literature.

COMPARE AND/OR CONTRAST THE EFFECT OF MEDIA ON AUDIENCE

COMPARE AND/OR CONTRAST THE EFFECT OF MEDIA ON AUDIENCE

ENC 1101 – Compare and Contrast Essay (1200 words)

For this essay, you will choose two articles that we’ve been assigned to read this semester and compare and/or contrast the effect of media on audience (see the chart below for available essays.)

The only exception is that you cannot compare and contrast the essays by Grisham and Stone.

You must provide a basis for comparison and utilize the Point to Point or Subject to Subject model.

In addition, you must include 2 outside resources from the library or library databases. These sources should provide some background or context for the comparison of your two chosen articles. One will be used in the introduction and one will be used for the conclusion.

Use MLA style for all formatting and citations.

Some important tips about writing academic essays:

Course essays should be formal. Formal essays should:

  • Avoid a conversational tone
  • Avoid the use of personal pronouns referring to the writer or reader of the essay (I, me, you, your, us, we, our)
  • Avoid contractions (shouldn’t, couldn’t, wouldn’t, can’t, won’t, etc.) – Simply spell them out.
  • Avoid web-based resources for support (Wikipedia, Sparknotes, or similar websites)
  • Avoid proofreading errors (be sure to spell check and read through your essay)

Some things to keep in mind about essays in this course:

  • If you do not meet the minimum word count, you receive an automatic 50% on the essay. The grade can descend from there. Since this course is Gordon Rule, it is important to meet the minimums.
  • When using any outside material, you must properly quote, paraphrase, summarize, and cite the material. If not, this can be considered plagiarism and result in penalties to your grade, such as a zero for the assignment. See the syllabus for details.
  • Be sure to review the Rubric for this essay to better understand the elements being assessed.

Democrats’ plans for health care reform or unethical scaremongering?

 Fra
How do you judge Palin’s quote? [“And who will suffer the most when they ration care? The sick, the elderly, and the disabled, of course. The America I know, and love is not one in which my parents or my baby with Down Syndrome will have to stand in front of Obama’s death panel, so his bureaucrats can decide, based on a subjective judgment of their level of productivity in society, whether they are worthy of health care. Such a system is downright evil.] Effective strategy
to oppose Democrats’ plans for health care reform or unethical scaremongering?
I think her strategy was both unethical and excessive, but it was effective.
She used false, unsubstantiated statements to affect a key health issue that
would have benefited a lot of patients.  The bill would have allowed Medicare to pay for optional doctor’s visits to discuss living wills and end of life issues with their physicians (Mason, 2016).  As a member of Congress, she and her family are afforded a higher level of health insurance that is effective for life after serving 6 years.  Due to her statements all references to end of life planning were deleted from the bill.
Reflect on what informs your judgment: commitment to advance care planning, analysis of facts, and/or political party loyalties?
I tend to be loyal to my political party but only if the facts support what I think is right.  I believe we should all be afforded the right to discuss end of life issues and living wills and have it covered by our insurance and not have our families burdened with these decisions on their own.
Is it right for nurses to endorse health reform legislation even if the legislation is not perfect? Does this apply to the recently failed American Health Care Act?
There is no perfect legislation so there must be some compromise.  I think nurses should endorse legislation if it is for the improvement of health care and in the best interest of the patients and nursing practice.

Evaluate the following methods of treatment and develop a treatment plan for Harvey: Alcoholics Anonymous, Transactional Analysis, and Family Systems Therapy

Treating the Substance Abuser and his Family Presentation

Harvey is married and has 2 teenage children. Harvey has been heavily drinking on a regular basis for the past 10 years.  In the past his wife often covered for him with his job, and usually made excuses for him with his children and his friends. Harvey has not attended his son’s baseball games this year, and he missed his daughter’s performance in the school play due to having been passed out from drinking. At this time, he is at risk for losing his job due to excessive absences, and he was recently arrested for a DUI. Harvey has realized that his drinking is adversely affecting all aspects of his life and he is ready to quit.
Directions:
Create an 8-10 slide Power Point presentation to discuss treatment plans for Harvey and his family. Use the speaker notes in the slide show to further discuss the topics on each slide. In addition make sure you have a title slide and a slide with references (in APA format).  Please reference at least two peer-reviewed journal articles in your presentation along with appropriate information from the assigned weekly readings. In your presentation:

  1. Evaluate the following methods of treatment and develop a treatment plan for Harvey: Alcoholics Anonymous, Transactional Analysis, and Family Systems Therapy. Please:
  • Provide  an assessment of Harvey’s situation and what he needs from treatment.
  • Evaluate and discuss the at least one pro and con of each of the following treatment methods: Alcoholics Anonymous, Transactional Analysis and Family Systems Therapy.
  • Develop a treatment plan  for Harvey:
    • Choose at least one of the above treatments for the plan.
    • Address at least two issues that Harvey needs to resolve in treatment.
    • Include at least three steps for Harvey’s recovery.
  1. Explain Brief Strategic Family Therapy and when it is used. Next, do the following:
  • Develop a treatment plan for Harvey’s wife.
    • Address her specific needs as the spouse of a substance abuser.
    • Include at least three steps for her recovery.
    • Take care that this plan is specifically addressing her recovery, and not how she can help Harvey recover.
  • Develop a treatment plan for Harvey’s children: Address their specific needs as adolescent children of a substance abuser
    • Include at least three steps for their recovery.
    • Recommend at least two substance use prevention strategies aimed at adolescent children of substance abusers.
  1. Choose one of the following cultures: Hispanic, African American, or Asian American, and analyze how being a part of this culture might affect Harvey and his family’s treatment and recovery.  Explain at least two issues present in treating substance abuse within that culture.

Abstract Objective To describe the models of practice used by nurse practitioners (NPs) and FPs in community health centres (CHCs), and to examine the roles of NPs and FPs in these models.
Design Cross-sectional study using an organizational survey completed by managers of the CHC sites, as well as administrative data on patient sociodemographic characteristics and encounter activities.
Setting A total of 21 CHCs (13 main sites and 8 satellite sites) operating in eastern Ontario during the period from December 1, 2006, to November 30, 2008.
Participants A total of 44 849 patients, 53 full-time equivalent FPs, and 41 full-time equivalent NPs.
Main outcome measures Family physicians and NPs models of practice, the sociodemographic characteristics and medical profiles of patients who were treated in each model of practice, and FPs and NPs use of time.
Results Patients were attributed to 1 of 3 models of practice in CHCs based on the proportion of visits to FPs and NPs: FP care (53% of patients), NP care (29%), and shared care (18%). Patients who received care in the NP model of practice were younger and more likely to be female, be homeless, and not have postsecondary education. Patients who received care in the FP model of practice had more complex medical conditions (cardiovascular disease, mental illness, lung disease, and diabetes) and more annual visits. Patients who received care in the shared care model had intermediate profiles. Nurse practitioners performed more off-site care and walk-in visits. Family physicians and NPs spent a similar proportion of time performing various duties such as direct clinical care and administration tasks.
Conclusion Although NPs mainly cared for their own patient panels (in the NP care model), they did share some patients with FPs and provide some care to patients under the FP model of practice. Patients who were cared for by FPs and NPs had quite different characteristics.
editors key points This study documented differences in patient profiles and care provision among the practice models of nurse practitioners (NPs) and FPs in 21 Ontario community health centres.
Nurse practitioners and FPs dedicated a similar proportion of their time to performing various face-to-face duties outside of the clinic but tended to be involved in different types of activities. Nurse practitioners provided more walk-in care and fewer same-day appointments than FPs did. Nurse practitioners also performed more street outreach functions. Nurse practitioner appointments were also slightly longer than FP appointments were. On-call services were considerably more likely to be covered by FPs than NPs.
Compared with FPs, NPs saw patient panels that were less medically complex but more socially complex.

Your assignment should be no more than 2-3 double spaced pages. Please note the 2-3 page limit does not include your instrument or references. Feel free to use more

space to create a well-designed instrument. You must type all assignments and make full use of spell check/grammar check features. We will not waste time trying to

interpret poor grammar. All assignments must be in APA format. We do not require cover sheets, but you must use appropriate section headings and references when

appropriate.

Grading
The assignment is worth 25 points. You will be graded on the description and appropriateness of your methodological approach, design, sample, and procedure. We will

also be grading on how well your instrument matches your research question and allows you to collect quality data that will help you to answer your research question.

As always, grammar, editing, and the general ease of reading in your assignment are important.

Directions
For this assignment, you will need to draft a plan and instrument for data collection. You should plan on collecting data from 10-15 people if you are doing a survey

and 3-6 people if you are doing an interview. If you plan on making observations, plan a minimum of 3 observations at least 5-10 minutes in length, but feel free to

make more or longer observations as well. Just keep in mind that you will have to code your observations for emerging themes, so it is important to only collect as

much information as you think you can handle.

Your assignment must include:

• Research question and hypothesis, with appropriate revisions from Lab#1
o Your paper must begin with your research question and hypothesis.
o Revise the research question and hypothesis if necessary according to your TA’s feedback from Assignment 1.
o Please note: You are not resubmitting your entire Assignment 1. We are only asking you show that you have incorporated the feedback you received on your

research question and hypothesis.

• Description of your selected research method AND design
o State whether your method is quantitative, qualitative, or mixed.
? Be sure to demonstrate correct understanding of this approach.
o Explain why you have chosen this method.
o State your chosen design (e.g., descriptive, correlational, experimental, case study, interview, focus group, ethnography).
? Be sure to demonstrate correct understanding of this design.
o Explain why this design is appropriate to answering your research question.

• Description of your preferred sample
o Describe who you plan to be in your sample
? Include all relevant participant characteristics (e.g., gender, age, college major, relationship status)
o Describe where you will locate your sample.
o Describe how you will recruit people to be included. This is separate from where you will locate them.
o Explain why this sample is appropriate for answering your research question
o Explain your reason(s) for being interested in this specific population.

• Create Your Instrument
o Create the data collection instrument you plan on using for your study. Do not simply describe it in words— you need to include an exact copy of the

instrument that you plan to distribute/administer to your participants. Be sure that all of the items on your instrument will ultimately help you answer your research

question. In addition, pay attention to how your instrument is formatted. It should be as easy as possible for your participants to answer.
o If you are doing a survey, include a copy of the survey you plan to use.
? Surveys should include 5-15 items.
o If you are doing interviews, include a list of your planned interview questions.
? Interviews should include 5-10 questions.
o If you are doing observations, include an example of what your field notes will look like.
? Field notes should include a description of the location of the observation, the time of day, the number of people, and what they are doing. You must include

all relevant information from the setting that you think will help address your research question. Be sure to include your thoughts about your observations.
o If you are doing an experiment, include your planned procedures and describe how you will measure your variables.
? Be sure to define your independent and dependent variables.
? Procedures must include step-by-step instructions for conducting the experiment, including designated groups, planned “treatments” or conditions, and

measurement instruments.
