The purpose of this assignment is to:

1. Identify and apply theoretical principles from a middle range nursing theory to the concepts of a phenomenon or situation in one’s clinical practice

2. Conduct a comprehensive library database search using appropriate controlled vocabulary and limiters and expanders to locate scholarly references related to the theory and phenomenon; and

3. Critically analyze and apply the theoretical principles to a nursing situation in a clinical environment.

4. Engage in writing to learn as a means to strengthen the written presentation of concepts and ideas essential to nursing scholarship.

Begin by identifying an area of specialty interest. From this interest recall an observation from your practice experience and write a short summary or scenario describing the nursing phenomenon or situation. Specify the concepts of the phenomena. Select one middle range theory that pertains to or explains the phenomenon identified. Next, specify the concepts of the theory that pertain to the phenomena.

Search a selected bibliographic database in the library to locate references pertaining to the concepts of the phenomena and theory and retrieve the most appropriate articles to reference in the discussion of the nursing situation. The body of the paper demonstrates a refined understanding of the nursing situation in light of the information from the journal articles and the theory. Primary sources are to be accessed.

The assignment concludes with a discussion of the concept, phenomenon, theory, and how it applies to his or her practice. The paper content should be nearly 5-7 pages (excluding the title page, reference page, and appendix) and prepared in APA style (levels of headings, double spaced, in-text citation, and references).

Comparative effectiveness research seeks to identify the most effective inter- ventions for particular patient populations. Meta-analysis is an especially valuable form of comparative effectiveness research because it emphasizes the magnitude of intervention effects rather than relying on tests of statistical significance among primary studies. Overall effects can be calculated for diverse clinical and patient-centered variables to determine the outcome patterns. Moderator analyses compare intervention characteristics among primary studies by determining whether effect sizes vary among studies with different intervention characteristics. Intervention effectiveness can be linked to patient characteristics to provide evidence for patient-centered care. Moderator anal- yses often answer questions never posed by primary studies because neither multiple intervention characteristics nor populations are compared in single primary studies. Thus, meta-analyses provide unique contributions to knowl- edge. Although meta-analysis is a powerful comparative effectiveness strategy, methodological challenges and limitations in primary research must be acknowledged to interpret findings.

Despite remarkable scientific advances over recent decades, the effectiveness of many health interven- tions remains unclear. The Institute of Medicine noted that evidence of effectiveness exists for less thanhalf of the interventions in use today.1 Scant evidence exists comparingmultiple possible interventions for the same health problem.2 Newer or more costly interventions may not be linkedwith better outcomes, and variations in health care expendituremay be unrelated to changes inhealthoutcomes.3-5The troubling lackof information about interventions’ relative effectiveness led to comparative effectiveness research (CER) initiatives.

CER can be defined as research designed to discover which interventions work best, under what circum- stances, for whom, and at what cost.1,6 CER methods include randomized, controlled trials; nonrandomized comparison studies; prospective and retrospective observational studies; analyses of registry and practice datasets; practice-based evidence studies; and meta- analyses.6-9 This paper examines using meta-analytic approaches for CER. This week, you study neurologic and orthopedic conditions with plastic surgery considerations. Both neurologic and orthopedic conditions are seen in the trauma areas of the hospital. These conditions encompass cardiovascular, pulmonary, and metabolic aspects of care in the ICU. EMS arrived and packaged him with spinal support. He arrives in the ER with breathing difficulty, chest pain, GCS 10, and O2 sat of 94%. You do initial assessment in the first 10 minutes and find that his chest is asymmetric for breathing and contusions are noted all over his chest. His CXR shows multiple rib fractures on both sides and pulmonary contusions. His cervical spine is stable in a collar. His thoracic and lumbar films are negative for fractures. In the next hours, what are your priorities, and what plan would you have for this patient as he moves to the ICU? In addition, after the acute situation is controlled and the patient awakens, what medication do you use for his agitation?

To prepare:

  • Review and select 1 of the 3 provided case studies. Analyze the patient information.
  • Consider a differential diagnosis for the patient in the case study you selected. Think about the most likely diagnosis for the patient.
  • Think about a treatment and management plan for the patient. Be sure to consider appropriate dosages for any recommended pharmacologic and/or nonpharmacologic treatments.
  • Consider strategies for educating patients and families on the treatment and management of the trauma care.

Post an explanation of the differential diagnosis for the patient in the case study you selected. Explain which is the most likely diagnosis for the patient and why. Include an explanation of unique characteristics of the disorder you identified as the primary diagnosis. Then, explain a treatment and management plan for the patient, including appropriate dosages for any recommended treatments. In addition, as a leader, you will be called upon to communicate recommendations and to lead a group through the course of action. What do Public Service leaders need to understand about this important component of the change process?

Greetings Milissa! One concurs with your sentiments in that many participants do enter the collaboration process not knowing what to expect, or even how to collaborate, and as such, the best way to manage this is to have explicit (rather than implicit) goals and a structure for the group in its proceedings. Hence the reason, it is one’s belief that the team leader should be an individual who understands the notion of, according to Malhi and colleagues (2009), the Care model, i.e. possessing an in-depth knowledge of: 1) continuing collaborative partnership, 2) rules which governs alliance an adherence to treatment, 3) customer service, e.g. relationship with clients, as well as, their families, and 4) client education and identification of warning signs. Moreover, in addition to understanding the above mentioned concepts, this leader should also possess assessment skills, in that, she/he must be able to 1) consider risk to others, which includes risk to children or other family members, 2) evaluate situation, e.g. giving consideration to social, occupational and cognitive functioning, 3) determine necessary treatment, or have some knowledge of it, e.g. if treated in the community, consulting with family ¿ carer to assist in taking steps to minimize risks and document these measures in a risk plan, and 4) exhibit degree of medical knowledge, in that, some medications can have significant adverse effects and require ongoing medical monitoring, thus, as the leader of the patient care team, being able to recognize changes in the client’s body/behavior is crucial. Thus, one would recommend that when selecting, electing or appointing team leaders, one should be mindful of the responsibilities that are characteristic of this position, taking into active thought that it is the obligation of the team’s leader to guide its members towards a solution which is based on best practices, as well as, the most ethical principles. Nubia, The enumerated steps are very enlightening and one cannot agree more with the contents of this post. The process of selecting members of the collaborating team is a very crucial one as you highlighted and this could very well determine the success or otherwise of the entire process and outcomes thereof. Laura Bronstein (2003) in prescribing a model for interdisciplinary collaboration between social workers and other professionals recommended highlighted the following five key components: ¿ Interdependence- which emphasizes teamwork. Here, there is the merging of expertise from different disciplines to achieve a common goal; ¿ Newly created professional activities-which refer to the creation of new processes which allow each professional achieve more within the collaborative effort than would have been achievable if the professionals were working independently, and helps evolve enduring structures; ¿ Flexibility- which refers to the deliberate blurring of roles that allows for constructive compromise that enhances team spirit and efficiency; ¿ Collective ownership of goals- which emphasizes shared responsibility in the process among the collaborating professionals; and ¿ Reflection on process- which refers to collaborators¿ continuous reference and commitment to the collaborative effort which goes on to enhance information sharing and feedback which strengthens the relationship Teamwork helps the professionals to merge their respective expertise to maximize creativity and also helps each professional to concentrate in their respective areas of expertise thereby improving the overall outcomes.
